Triumph Construction failed to properly support a trench before a slab collapsed on two workers, OSHA officials say.

OSHA found workers were exposed to another potential collapse in the same trench a month after the fatal incident.

There were 22 trench-related deaths in the first half of this year, up from 15 in all of 2021.
